The 2008-2009 Registration is a four step process that must be completed by November 11, 2008:
1) Click here to renew your US Lacrosse membership. You will need the current membership number and the renewal date during the online registration process.
3) Print the following forms and mail these to the registrar listed below with your check for the NON-REFUNDABLE commitment fee of $100 payable to "Foothill Lacrosse Booster Club" (if you have not already paid this fee):
4) Click Here Go to the Foothill High School website and complete the required district forms. NOTE: These forms are NOT required again if they are on file for a Fall/Winter 2008 sport.
